The relaunch of the Cuyahoga County Transition Collaborative (CCTC) will strengthen a collaborative network focused on coordinating services and supports for students and young adults with disabilities as they transition to adult life. As a multi-agency planning organization, the CCTC is committed to providing agency-neutral support that promotes collaboration, shared problem-solving, and access to resources. CCTC meetings will bring together school districts, the Cuyahoga County Board of Developmental Disabilities (Cuyahoga DD), Opportunities for Ohioans with Disabilities (OOD), along with other community service providers to strengthen partnerships, reduce barriers, and improve coordination across systems. Through this collaborative approach, partners will work to align services and resources that support meaningful employment, postsecondary education and training, and independent adult living.
