The Cuyahoga County Transition Collaborative (CCTC) is a multi-agency organization dedicated to providing agency-neutral support and services to youth and adults with disabilities.
The CCTC meetings provide an opportunity for transition planning team members to collaborate. CCTC meeting agendas include topics such as, supported decision making, coordination of services, multi-agency planning, person-centered planning, and other relevant “good life” planning topics.Fall Meeting Topics
Multi- Agency Planning and alignment of Individual Education Plans (IEP), Individual Service Plans (ISP), and Individual Plans for Employment (IPE).
Development of the CCTC vision, mission, and plans for including other agencies, such as Children and Family Services, Department of Youth Corrections, Parent Mentors, Youth Opportunities Unlimited, and individuals with disabilities.
Winter Meeting Topics
Review CCTC vision, mission, expansion of an organizational structure for sustainability of the collaborative.
Providing agency-neutral services based on person-centered data.
Share out by member agencies (ex: Opportunities for Ohioans with Disabilities Summer Youth Work Experiences).
Spring Meeting TopicsSmall group case study work with redacted IEPs, ISPs, and IPEs.
Finalize the CCTC vision, mission, goals, and strategies using the OCALI Multi-Agency Planning (MAP) Tool.
